Normal Operations[]

Normal Operations are done under normal circumstances without any influence from the level.

Following Operations can be done in normal mode:

During Corridor Operations, Nigel operates in a hallway, running with Bob through a seemingly infinite string of corridors, and sometimes going outside. Bob lies on a constantly moving bed (Heart, Double Kidney, Teeth transplants), or sits in a wheelchair moving backwards (Brain and Eye transplants) depending on the operation, while the tools and new organs are on separated tables that move in and out of the operation frame.

The carts with the tools and organs are randomized and stay beside the table/wheelchair for approximately 15 seconds. Because of this, there can be multiple tools of the same kind and even multiple organs. Dropped organs and tools will also respawn onto the carts in the next cycle. However, it can be frustrating since the tools and organs may not be available when necessary due to RNG. A good method to get around this if the patient is in a bed is to drop more necessary tools (particularly the green syringe) on the patient's chest around the neck area. The wheelchair has a built-in table to place tools.

While moving, a set of double doors is slammed open. This causes the screen to shake, but does not affect Bob or Nigel.

Just like in the Normal Operations the standard operations, as well as the new previously Surgeon Simulator Touch exclusive operations are available:

Notes[]

'Nothing But Skull' and 'Let's See That Sick Filth Again' achievements are best done in this mode because if the player loses the brain/spoon, the player can get another one.

Trivia[]

  • On the Anniversary Edition, when outside of the hospital, there are rain particles.
  • The coloured borders on the floor inside the hospital change colour when Nigel goes through an outdoor section, and into an indoor section.
  • The audio will change distinctively when approaching a door.
  • When the radio is knocked offscreen, the music will slowly pitch and slow down.

Ambulance Operations[]

The Ambulance Operations are unlocked after beating each normal surgery, and unlock Space Operations after completion. These operations are included in both Surgeon Simulator, and Surgeon Simulator: Anniversary edition. In this mode, the player performs surgery in a poorly driven ambulance.

Hazards[]

As each Ambulance operation starts, the ambulance will start too, and will drive extremely fast, sometimes in which will result in a bump. The ambulance will also suddenly go left and right, as like the driver was trying to avoid hitting other cars. In mid-rides, the ambulance might slow a bit down, and accelerate again. All of these scenarios are randomly picked and each cause Nigel's hand to shake around, among with the physical objects inside. The Ambulance's Back doors can swing open, and close themselves, due to speed. In random situations, the doors will swing open, allowing various things to fall out onto the road. After a few seconds, the doors will close again. The doors can also render each operation impossible to beat. Organ(s) can fall out if in the right spot, forcing one to have to restart the entire operation over again.

Trivia[]

  • The ambulance operations are almost impossible to beat at certain cases, because the chances of losing the required tools or the required organ(s) is high.
  • If you listen closely, you can hear chatter from a radio.
    • You can also hear the sounds of cars honking, the sound of the headlights blinking, the screeches of wheels in the background, and the ambulance's siren.
  • If you throw The Clock out of the ambulance doors, the music slowly fades away and changes pitch.
    • This proves the clock plays the music heard in each operation.
  • It is recommended you throw a dangerous tool out of the ambulance's door after you finished using it, because the ambulance will make a sharp turn, and will cause that tool to fall into Bob's stomach, or if you are doing other operations, like the Brain Transplant, Eye Transplant, and Teeth Transplant, it will most likely bash into Bob's head, both causing a lot of blood loss.
    • Such objects include, but are not limited to, the hammer, fire extinguisher/oxygen tanks, and the axe.
  • In uncommon situations, the green Syringe may inject itself into you if your arm is at the right place at the right time, causing hallucinations.
  • The ambulance operations are referenced in the official Barnardshire General website.
  • Organs are prone to clipping into Bob during a turn.
  • The ambulance driver was possibly supposed to talk to Nigel, with cut voice lines still being in the game as audio indicators for the hazards.
    • He refers to Nigel as "Nige"

Space Operations[]

During the space operations, the player operates within a small space station in outer space. The patient is strapped to the bed so he cannot float and remains stationary.

Because there is no gravitation, the tools and organs fly around the station during the operation and can't be attached for easier access. This does not have any influence on the players hand or the patient.

It is easy to lose some of the more important tools or organs by pushing them above the player where they cannot be reached. If this happens, waiting is required to retrieve the lost organs.

Be very careful that the green needle may float and poke you. If the green needle pokes you, find the blue syringe. And be careful to not let the blue syringe touch the patient, as it will increase blood flow.

Going to the number pad in the Space Brain Transplant mission and entering "296145" triggers the special location Outer Space, in which the Alien Surgery takes place.

Special Operations[]

There are three special operations which have to be unlocked by the player to be played. After unlocking them, they can be played by inserting the VHS cassette into the VHS player under the TV-System with the static display.

Completing each of the operations rewards the player with statues that resemble the operation.
